What is happening in the business that brought this conversation to the surface.

A specific event, a pattern you have been noticing, a strategic shift. Whatever comes to mind first.

Select all that apply. This helps me understand the texture of what you are working with.

Lost revenue, missed strategic priorities, turnover, slowed execution, something else. A best guess is fine.

The Leadership Picture

Who the leaders are and where you see the most need.

Programs, coaching, assessments, off-sites. What worked, what did not. If this is the first formalized effort, just say so.

No need to be eloquent. Plain language is best.

Success and Stakes

What good would look like, and how urgent this is.

Try to get to one or two specific business signals. Revenue, retention, decision speed, employee survey scores, time-to-productivity, something concrete.

Approaches that did not fit, vendors you have already considered, anything off the table.

Decision and Logistics

A few practical questions so we are aligned on how this might move forward.

Names and roles are fine, or just roles if names are sensitive.

Multiple viewpoints often surface patterns one person cannot see alone. Each invitee will receive their own version of this questionnaire. Their responses come to Dan, not to you, so they can be candid.
